Subject - EGC common mistakes

i will like inspectors and electricians do list common mistake with EGC related issues.
because i want to review this and this topic somehow can get misunderstood with NEC and local issue related items.
so go ahead and make the list and i want to see where we go from here.
[ i will admit it here i have some troble with EGC related items and read nec sec 250 area and feel little confusing so i will like to get it straghtend out in here ]

| i read this one from another site.....when we go to a larger ungrounded conductor for any reason we adjust the EGC with 250.122b.....so if we need to replace the single service cable with 2,,,aka, parrelle conductors,,,,how do yall go about sizing the EGC?

| Why would you be replacing the cable with 2? If you install two 2 parrallel conductors that are sized properly, 250.122(B) won't apply, because you are not increasing beyond the normal minimum size. If you had 1 1250Kcmil and decided to replace it with parrallel 750's 250.122(B) would apply, in my opinion. :)
